The first is lead volume without lead quality — an agency reports hundreds of "leads" while the calendar stays empty, because a lead is defined as anyone who clicked a form, not anyone who showed up. The second is the disappearing act: responsive through the sales process, unreachable once the retainer clears. The third is the locked account — the agency runs ads from its own ad account and its own pixel, so when you leave you lose the audience data, the conversion history, and the campaign learning you paid to build. The fourth is spend markup, where the agency bills you for ad spend at a rate above what the platform actually charged.

None of these require an agency to be malicious. Most are just structural incentives nobody disclosed to you at signing.

How the 'guaranteed appointments' bait actually works

One owner summed up the ads that hunt her better than any marketer could: "There are so many stupid ads on Social like 'we guarantee 30 appointments a day or you don't pay' and 'marketing firms' that 'figured out the algo' for med spas, and will make you 8 figures in 2 months. I know it's BS but I also need direction."

That instinct is correct, and the mechanism is worth understanding. A guarantee is only as real as its definitions. If the contract does not define what counts as a qualified appointment, who verifies it, and what specifically happens when the number is missed, then the guarantee is a headline, not an obligation. Common escape hatches: counting a form-fill as an appointment, counting a no-show as delivered, requiring you to maintain a minimum ad spend to stay eligible, or offering a remedy of "continued service" with no time limit and no refund.

3. Ask what a lead is defined as, then ask what a booked consult is defined as, then ask which of those the reporting is based on. If the answer to the third question is "leads," you will never be able to tell whether it worked.

4. Ask whether they mark up ad spend, and ask to see a platform invoice alongside their invoice for the same month. You pay the platform directly at a properly run agency.

5. Ask whether they work with your direct competitors. There is no right answer to this universally, but there is a right answer for you — and an agency running three clinics in your ZIP code is optimizing across a portfolio you are only one part of.

The reference check nobody does

Ask for two references: one current client, and one who left. The second request is the one that separates agencies. A confident operator will give you a name and let you ask why the relationship ended. An evasive one will explain that former clients are not available for reference — which tells you what you needed to know without the call.

When you get the current client on the phone, ask a specific question rather than a general one. Not "are you happy with them?" but "what is your cost per booked consult, and do you know it off the top of your head?" If a real client cannot answer that, the reporting is not working regardless of how they feel about the relationship.

What a fair arrangement actually looks like

You own the ad account, the pixel, the audience data, and the creative. You pay the platforms directly with no markup. The reporting shows spend, leads, booked consults, cost per booked consult, and show rate — the last two being the only ones that indicate whether the machine works. There is a defined exit: a notice period, and your assets stay yours.

None of that is exotic. It is simply what an agency looks like when its incentives are pointed at your calendar instead of at your retainer — and it is why the owners in those threads are angry, because so few of the arrangements they signed contained any of it.
